Memorial Hermann Release of Information Department is developing a requester portal which will allow you the ability to manage your requests online at times that are convenient for you.
Access to the MH Requestor Portal will be available by accessing our Memorial Hermann website and creating a user profile. Once you have registered as a user, you will be able to create your own tracking board to assist in managing your requests by adding new requests and removing completed requests. For each request, a portal code will be generated and provided to you. This portal code is mandatory for you to access any information regarding your requests on the MH Release Portal.
