Memorial Hermann Release of Information Department is developing a requester portal which will allow you the ability to manage your requests online at times that are convenient for you.

Summer 2016
Phase I: Status checks and online payment

Access to the MH Requestor Portal will be available by accessing our Memorial Hermann website and creating a user profile.
Once you have registered as a user, you will be able to create your own tracking board to assist in managing your requests by adding new requests and removing completed requests.
For each request, a portal code will be generated and provided to you. This portal code is mandatory for you to access any information regarding your requests on the MH Release Portal.
The tracking board will allow you the following features:

  • View submitted request;
  • Review all correspondence from Memorial Hermann including the acknowledgment letter and invoice;
  • Track the status: in process, payment pending, released, USPS tracking number.
  • Online credit card payment feature
