These FAQs will help provide a better understanding with registration and/or login to the Memorial Hermann Release of Information Requestor portal. This document also provides some troubleshooting steps to fix common issues with registration and/or login.
1. I registered but could not log in
Once you have registered as a new user with an Email, Password, and Confirm Password, please go to that email account Inbox and check for an email coming from "".
Please note that it may take anywhere from a few seconds to an hour to reach to your Inbox. Also, please make sure to check your Junk e-mail folder frequently.
Once the email arrives, click on the link "Click here to complete your registration" and enter the password that was used to originally register. You will then need to click on the validate button.
This will take you to the form where you can enter your details and set up security questions.
Click "Register" once completed and it will take you back to the main page where you will now enter your email address and password to Log In.
2. I did not receive an email after registration
Please note that it may take anywhere from a few seconds to an hour for the registration confirmation email to reach your Inbox. Also, please check your Junk e-mail folder frequently.
Please verify that the password you entered meets all of the security criteria (check marks will appear after each criteria is met).
3. I still did not receive an email after one hour
  1. Go to main page and enter your username and password that you originally registered with under Registered Users. Please make sure you enter correct password.
  2. Click "Log In"
This will resend an email to complete the registration process. Please wait 15 minutes and keep checking your Inbox and Junk e-mail folders. If you do not receive an email, please follow Step 1 and 2 and retry up to two more times. If after four tries, you still did not receive an email to complete registration, then Contact Memorial Hermann Release of Information.
4. Any recommended browsers?
We recommend Internet Explorer 10 or above or latest version of Chrome. However, the requestor portal should work with all major browsers.
5. I click on the email link for registration, enter the password and then I click the Validate button. Then I receive "An error occurred when attempting to complete registration"
This could happen when the password does not match with what you had originally entered during registration.
If you remember your password, click on the email link once again and try entering the correct password.
If you forgot your password, Go to main page and click "Forgot your username or password" to reset your password. A new email will be sent to the registered email account. Please then click on the email link to reset your password. You may then go back to the Login Page and enter your new password
6. Any Cookie settings to watch out for on the browser?
On Internet Explorer
  1. Open Internet Explorer
  2. Click the Tools button, and then click Internet Options.
  3. Click the Privacy tab, and then, under Settings, move the slider to Medium or above, and then click OK.
On Chrome
  1. Click the menu icon on the browser toolbar
  2. Select Settings
  3. Click the Show advanced settings
  4. Click Content Settings
  5. In the "Cookies" section of the dialog that appears, make sure Allow local data to be set is selected to allow both first-party and third-party cookies.
  6. Click Close
7. MHR number failed to add to tracking board
Please allow 3 hours from the time you received your acknowledgement letter to add the request to your tracking board.
